A few things to know before you start:
Both the participant and a parent/guardian signature are required.
Adult leaders register using this same form, filling it out as the participant. If both spouses are participating, one adult may sign both signature boxes.
If your youth has a food allergy, dietary restriction, medical need, or physical restriction, answer “yes” to the relevant question on the form and explain in the text box — see the FAQ page for details on how this information is handled.
